Introduction
A form on an information page lets you collect structured information from multiple users in one fixed location, for example during onboarding or for recurring requests. Unlike a form in a message, a form on an information page stays permanently available.
In this article you will read how forms on information pages work, when to use them, how to add a form, and what to do if you run into problems.
You need an content-manager role to place a form on an information page. Regular users can only add a form to a message.
How do forms on information pages work?
A form on an information page is intended for structural information collection: situations in which you repeatedly need the same data from different users.
For each form you decide:
- How many questions the form contains.
- Which answer type applies to each question, for example text, multiple choice or date.
Visitors of the information page can fill in the form as soon as they open the page. All submitted answers are available in one central location.
When should you choose a form on an information page?
A form on an information page is the right choice when:
- The form is not a one-off.
- Multiple users need to fill in the same form.
- The information must always be available in the same place.
Use an information page with a form for onboarding new employees, recurring requests or periodic checks. For a one-off request, a form in a message is more practical.
Adding a form to an information page
Follow the steps below to add a form to an information page.
- Create a new information page.
- Give the page a name and add the desired text.
- Scroll to the bottom of the page.

Click Form.

A selection screen appears. Choose whether you want to create a new form or duplicate an existing form.
Duplicate an existing form
Choose Duplicate existing form when:
- You already have a form similar to what you need.
- You want to reuse questions from an earlier form.
A copy of the existing form is created for this information page. Changes to the copy do not affect the original form.
Create a new form
Choose New form when:
- You want to start from scratch.
- The form has a completely new purpose.
An empty form is created in which you add the questions and answer types yourself.
Troubleshooting
I don't see the Form button at the bottom of the information page
- Check that you are logged in with an administrator role.
- Make sure you have opened the information page in edit mode.
I cannot select an existing form
- Check whether forms have already been created in the environment.
- The selection screen only shows forms you have access to.
The form is not visible to other users
- Check that the information page has been published.
- Check that the page is visible to the correct target audience or user group.
