Introduction
Forms make it easy to collect feedback, send out questionnaires and gather information from your colleagues. You can add forms to messages, pages and to items in the Programme manager.
In this article you'll learn which four types of forms are available, how they work, how to create one and what to do when you run into problems.
Forms are available in both the mobile app and the browser version of Plek. You can share a form with everyone on the platform, with a selected group of users or within a specific group.
How forms work
Plek supports four types of forms. Each type has its own purpose and its own set of features. Choose the type that best fits what you want to achieve.
- Poll: a short, anonymous vote. Results are shown in a chart immediately after submission. Use this for quick opinion polls, for example about new office furniture or dietary preferences for a team event.
- Simple form: a linear questionnaire without branching logic. You have access to all question types and can group questions into sections. Use this for straightforward questionnaires, such as a short evaluation.
- Advanced form (recommended for complex cases): includes everything in the simple form and adds question branching. Users are routed to different follow-up questions or sections based on their previous answer. Suitable for layered questionnaires such as intake forms.
- Quiz: tests users' knowledge with automatically scored questions. Question types are limited to those the quiz can grade itself. Afterwards you can show a score or feedback. Suitable for evaluations after training sessions or onboarding modules.
A Poll and a simple form can always be upgraded to an advanced form later. The other way around is not possible.
Not all question types are available in a Poll or Quiz. If you need full flexibility in question types, choose a simple or advanced form.
Creating a form
Follow the steps below to create a new form. The steps work the same way in the browser and in the mobile app.
- In the menu, go to Forms.
- Click New form.
- Choose the type you want: Poll, Simple form, Advanced form or Quiz.
- Enter a title and description.

Add your questions and adjust the settings per question (for example whether a question is required).
- Decide who you share the form with: the whole platform, a selected group of users or a specific group.
- Click Save. Your form is now available to the chosen audience.
Want to create a similar form quickly? Duplicate an existing form and only adjust the questions that need to change.
Troubleshooting
I can't see the results of my form
- For a Poll, results are shown immediately to everyone who submits the form.
- For a simple form, advanced form or Quiz, you need the right permissions to view the results. Ask your administrator to check your permissions.
I can't select a particular question type
- Polls and Quizzes don't support all question types.
- Choose a simple or advanced form if you need full flexibility in question types.
My form needs to use question branching
- Question branching is only available in an advanced form.
- Currently using a Poll or simple form? Upgrade via Settings > Upgrade to advanced form.
Users need to submit the form multiple times
- This option is only available in an advanced form.
- Enable it via Settings > Allow multiple submissions.
My problem isn't listed here

Additional functionality
A few extra features that can come in handy when working with forms:
- Duplicate a form: existing forms can be copied as a starting point for a new form.
- Show previous answers: in an advanced form you can choose to keep previous answers visible when a user returns to the form. This way someone can complete or correct their submission later on.
